This think sheet doubles as a reflection for students on their choices, as well as a parent note home. Students complete the form, indicating what expectations (safe, responsible, respectful, kind) they had trouble meeting, explaining what happened, and how they will do better the next time. The student and teacher sign the form, and send home to be reviewed and discussed between the parent and student.
I use this sheet in my own 1st grade classroom as a way to guide my conversation with students who are making repeated mistakes, and then to reinforce our discussion by looping parents in to our plan (what they will do differently next time.) This has been a great tool for me in my classroom, as I hope it is in yours! I print these on neon yellow paper so parents can't miss it (and sometimes send a message to expect it in their backpacks, just to make sure).
