One of the most difficult parts about parent outreach is keeping track of the phone calls, emails, and texts to students' families. This customizable parent outreach log/contact form allows you to document all parent outreach and the outcome of each conversation.
You can enter data into -- and sort this document by -- the following fields, all of which are customizable:
* Last Name
* First Name
* Language (useful for ELLs)
* Phone Number
* Medium (phone, text, email, Skedula)
* Reason for call (attendance, academics, behavior, general outreach)
* Answered (yes, no, left message)
* Spoke with (parent, student, other, N/A)
This parent outreach log can be used on Microsoft Excel, Google Sheets, OpenOffice, and any other programs that can read .xlsx files. If you use this file on Google Sheets, you can even share it with the other teachers on your team and keep all of your outreach in one place.
You can also use this document to:
* document outreach to students
* determine how many times you have called home about a particular issue
* see how often you call the same students
* see how often you make positive parent outreach
Because this outreach log is accessible on so many platforms, including mobile devices, documenting parent outreach will be the easiest thing you do all day!
