As a high school teacher I am always thinking of ways to engage parents in a way that is grade appropriate. I created this questionnaire to keep myself sane and engage families from the very start. It's a quick, easy way to let families know you are there for their children while also gauging what they expect from you in regards to communication. This allows parents to reflect on how involved they want to be in the classroom. I send this sheet home at the beginning of the year with stamped envelopes in an effort to get more of them back (a great bonus if your school will pay for the postage).
The questionnaire is my first step in engaging parents and families in their child's education. It opens the path to consistent and clear communication and also allows for you to attach your contact information or any information about an Open House or Curriculum Night that your school may be hosting. I would use this form with any grade!
