Description

This survey is an editable Google Form for you to give to parents to learn more about your students and their families. There are four parts to the survey:

1) Parents/Guardian Contact Information

2) Student Information: asking questions about students strengths, weaknesses, attitudes towards school, and prior language learning experience

3) Family Spanish Information: asking parents if they speak Spanish or have lived in a Spanish-speaking country and whether they would be willing to serve as a guest speaker in class

4) Classroom Expectations: requiring parents to check that they understand expectations for the proficiency-based classroom. (You of course can edit these to include expectations about your class homework, grading, tardy, and absent student policies.)

This is a wonderful way to build parent relationships, learn more about your students, gather guest speakers for the school year, and manage parent expectations about your classroom. Any section in the form can be edited, added to, or deleted to meet the needs of your classroom.
