I have created these beginning, middle, and end of the year parent surveys in order to help overall parent communication. This is to ensure parents that they do have a say in things and their opinions do matter. So I have created these surveys to help us grow as teachers based on the feedback we get from our student's parents.
The Beginning of the Year Survey:
Does your child enjoy reading? If so, what do they like to read?
What are your child's strengths?
What are your child's weaknesses?
Do you have internet (e-mail) access?"
Any other information I need to know? (Health, Allergies, etc...)
The Middle of the Year Survey:
Are you happy with your child's reading progress? Why or why not?
Do you feel informed enough about what is going on in your child's classroom?
How can I help your child succeed even more in the classroom?
What is working well for your child?
Is there anything you would like to see changed or improved?
The End of the Year Survey:
Are you happy with your child's progress from the year?
What did you think about the homework?
What is something I did that made you happy? What is something I did that did NOT make you happy?
Are you satisfied with our overall communication?
I have also put a quick letter to the parents explaining the survey and thanking them.
All of the TEXT is editable, so if there are any questions you don't like, please feel free to change them so it meets your needs best. :)
