Parent Teacher Conferences coming up? Well, here are two forms that will help your conference go smoothly. These forms will help make sure your parents leave knowledgeable of their child's performance in school and walk away confident of helping their child get better.
The first page is a form to give BEFORE the conference. It asks the parents what topics they would like to discuss with the teacher...from subjects, to behavior, to how the child gets along with others. Knowing what your parents want to know will ensure a more productive conference.
The second page is a form to give AFTER the conference. It is a summary/checklist of ways the parents can help with their child's progress. This form can be filled out prior to the conference, or it can be filled out during the conference. It is basically a checklist of goals and at-home practices that the parents can work on to help their child improve.
