How many times have you attended a conference, wrote all of the notes and info down in a notebook....and then it wasn't legible, you forgot something, someone else wanted a copy, etc.?
I created this form so that this is not an issue anymore. It includes a space for all relevant info (date, student, parent/guardians in attendance, teachers in attendance, reason for calling conference, other times communication occurred) as well as a large space to take notes on what is discussed. This form could be easily filled out and then copied for everyone that was present at the conference.
