Parent-teacher conferences can be stressful. This is great for new teachers to keep excellent, detailed records. The document contains spaces to record the following:
The words academic/behavior to circle reason for meeting, student name, the date you recorded child's grades on document, grade/percentage for reading, language arts, math, science art, and social studies, notes on what you are observing in class, suggestions to the parent/guardian, parent name, parent signature, date of meeting (today's date), teacher signature, and a parent/guardian confirmation stating that you provided them with a copy for they could take home and review.
Page 1 does NOT include art for the subject areas.

Helpful hint: The date on top does not have to match date on bottom. For example, let's say on Sunday you decide to fill out a parent-teacher conference form because on Tuesday you have a meeting scheduled to meet with a parent, you will record Sunday's date on top. Why? Because you are looking at his grades on Sunday. They might change come Tuesday. This way you are not giving false information. And if the parent shows up for the meeting on Tuesday you will write Tuesday's date on the bottom under parent name or the parent can just write in the date. :)
