Parent-Teacher conferences can involve a lot of discussion and information about what is seen in the classroom. The 3-page conference form provides an opportunity to cover all subject areas with a quick check sheet as well as a section that covers behavior and work habits. Websites are also given as a tool for at-home practice, as well as a section to write in additional comments. In the past, I have used this form a few months after the beginning of the year to discuss the child's progress at that point.
When completed before the conference and photocopied for the parent, the form provides a very thorough and organized layout for the conversation of the child's progress in the classroom.
