It is that time of year again...Parent Teacher Conferences!!! I have created this pack of everything you need for a smooth conference. This file includes the following. Please remember that you do have to insert your own names, information, and dates. This is just a template.
Initial Sign Up Letter: I send this letter home first and let the parents choose 3 different times. As they come back, I number them and I assign times on a first come first serve basis.
Conference Time/Parent Survey: After I make the schedule, I send home this form to let the parents know their time. It also has a survey so to give me a better idea of what the parents would like to talk about.
Progress Report: I fill out this form for every student. I check boxes that apply to them and I write the student's strengths and weaknesses. I also fill in grades and testing scores. You can change the tests to any that you use. I use this during conferences as a guide to make sure I cover everything I want to and don't forget anything.
"I Can" Checklist: Every nine weeks, I make a checklist of all the skills we have covered. I check all the skills that the student has mastered. This will give the parents a good idea of what their children need to work on.
Reminders: Send these home the day before conferences.
Sign In Sheet: Have the parents sign in to document who showed up.
Also, I am going to give each parent a little bag of popcorn that says "Thanks for popping in!" These tags are included!
