Parent-Teacher Conference Forms
Getting reading for Parent-Teacher Conferences can be a stressful time. These forms, though, can take away the stress of scheduling Parent-Teacher Conferences as well as make the conference time more productive and more informative!
All of the forms were created in Microsoft Publisher and can be completely edited and customized for you. I used red ink to highlight specific areas that need to be changed by you. These areas include: dates, times, teacher’s name, room number, phone number, etc.
Parent-Teacher Conference Scheduling Form: Send this form home about two weeks in advance. This gives you time to send home the confirmation letter.
Confirmation Note for Parent-Teacher Conference: Send this note home with the student and don’t forget to note the appointment on the schedule!
Parent-Teacher Conference Schedule: Use this form to schedule the Parent-Teacher Conferences. Turn the complete form into the office. This lets the office cancel/reschedule appointments for you—if needed. Post a copy in the classroom window or type up a daily appointment schedule to post in the window. (I like to type up five daily appointment schedules, one for each day of the week, and post all five in the window. This serves as a reminder to parents when they are picking up or dropping off their children.)
Parent-Teacher Conference: Use this form to help guide the Parent-Teacher Conference. Speaking for myself, I sometimes forget important information or test results I want to share with the parents. This form keeps all of those important key points right at hand.
Parent-Teacher Conference Student Survey: Send this form home with the Parent-Teacher Conference Scheduling Form. Encourage the parents to fill it out and bring to the conference. You will be amazed at what you learn about the student!
