This packet is a great resource to have prepared before a parent teacher conference for those students who are struggling in class or just for those teachers who like to have a structured, detailed conference. It would also be a great resource for those teachers that like to do student led conferences.
Included with this resource is a self evaluation that the student can fill out in class before their parents come in for a conference. This will be helpful for the parents to see how their own student sees themselves.
The next page is for parents to fill out. Parents need to have input and often see things at home that we as teachers don't see. This gives them a chance to share those things with us.
Of course there is a teacher evaluation form for the teacher to fill out ahead of the conference.
There is also a goal plan for the teacher and parent and possibly student to collaborate on what would be appropriate goals for the student to accomplish and what the incentives or rewards would be for reaching those goals.
