Parent Teacher Conference Forms | PRINT & DIGITAL
What educators are saying
Parent teacher conferences can be a stressful time of year so I created a few FREE printables that will help you plan ahead and be prepared.
This resource includes:
- Pre-conference questionnaire
- Student self-evaluation
- Student conference summary forms
- Conference folder covers
One of my favorite ways to plan and prepare is by sending home a pre-conference questionnaire. Ever since I have stared doing this, I am no longer caught off guard by an issue or question that I haven't prepared myself to answer. I give my families plenty of time to respond to the questionnaire so that I receive quality content from them.
Prior to conferences, I ask my students to complete a self-evaluation. I like it because it makes the students aware of the different topics that I will be sharing with their parents. It also serves as a nice conversation starter for areas that need improvement.
I like to use this conference summary freebie for my parent teacher conferences. It provides a nice template for the families to look at as an overview of their child's academic performance. This freebie includes a boy template and a girl template. The summary boxes include math, reading, writing, and spelling.
Do you like your conference folders to look nice for your families? This is a quick and easy way to do just that! This FREEBIE includes a boy and girl conference folder cover page for grades kindergarten through sixth grade. Simply print and glue to the front of a folder or 9x12 piece of folded construction paper.
