My students always have such a blast completing this Who Am I form. It has clues about the child and then they draw a self portrait. They leave it face-up on the desk and when their parent comes to parent teacher conference they have to search for their child's desk by reading the riddles. Once they find it they can turn the paper over to check if they were right and the student has written them a letter of appreciation for supporting them in their education on the back.
Before writing the letter we talk in class about all of the ways parents support their child; like bringing them to school, helping with homework, reading with them, buying clothes, supplies, lunches, etc. The kids love it and love surprising their parents with a note of appreciation and love. One parent read it and cried tonight at PTC. :)
If the parent comes to PTC without their child I give them the Letter to my Child form to complete and leave on their desk in place of the original Who am I form. If the child comes with their parent I just send the form home with the parent to give to their child whenever. Hope you enjoy! Please let me know if you have any questions.
