This form is a concise report that give parents information on their child's beginning of the year test scores and also includes a checklist of work habits. I also like to include a checklist of a few academic and behavior strengths and needs for each student. I have included some sample statements that I use for strengths and needs. It provides a lot of data on one sheet and doesn't take too much time to fill in. I have created this on a Word document so that you can easily edit it to suit your needs.
