I am a little old fashioned when it comes to sign ups for my class. For a couple years I tried using an email blast for parents to sign up for conference times, and I used a sign up program I found on the internet once, but neither produced results for me like sending home a hard notice. I emailed out a notice that it was coming home two weeks before my due date, reminded parents in my newsletters, then sent a final email the night before it was due. This helped greatly and I usually had meetings with a majority of my students' parents. (I hate to admit, but for the past two years I didn't take appointments past my due date as well.)
I left this file in Word format so that you can customize your dates and times. I also used a copy of this form as a master copy for my own schedule. When I got the student's form I would highlight their appointment time and write, "This is your time" next to it. I would record the parent and student's name on my master and hand the form back to the student to give their parent.
