I, like most teachers I know, like to plan ahead way in advance and allow their students' parents to choose a conference time that works best with their schedule, which can then help boost parent attendance at conferences! It's a win-win! Therefore, I set this form out in my classroom during my school's open house at the beginning of the school year for parents to complete. Then, a couple of weeks before parent teacher conferences are scheduled to take place, I follow-up with my students' parents to confirm or change their conference day and time.
The first form in this document is left blank with the intention that your students' parents can complete it without assistance. However, the second form provides an example of how they can complete it without direct instruction from you, as back to school and open houses are very busy times for us teachers!
