As an Itinerant, I am often the primary teacher and/or Case Manager for my Deaf and/or Hearing (DHH) students; therefore, I am the responsible party for coordinating and scheduling student Annual and 3 Year Re-Evaluation IEP Meetings. This can be a daunting task, especially when you are providing DHH Services to multiple school districts. In order to further assist myself in collecting parental and/or guardian feedback, prior to the IEP Meeting, I always send out this personalized Parental Questionnaire. It should be noted that I ALWAYS include a self-addressed envelope with paid postage with the questionnaire so that the parent and/or guardian can complete the questionnaire and put it in the mail with ease. This allows me to identify AHEAD of time IF they have any concerns or questions that should be addressed prior to the IEP Meeting, as well as gives me adequate time to contact the appropriate parties (i.e., administrators, teachers, specialists, etc.) to find answers to the parent/guardian questions. It also ensures that I have administrative support present at the IEP Meetings for items that could cause problems for individuals at the IEP Meeting.
