We have all had that day when a student approaches to inform us (usually long after the fact) that they have done "all of the work" on an assessment involving a group or partner. Well this form could be the perfect solution!
This form gives students the opportunity to quickly provide feedback to the teacher about their experience working with another student or students. For some students it can be difficult approaching the teacher to let them know what is really going on. Sometimes it is easier for them to write something down rather than approach you face-to-face. This form gives the student a more comforting method of communication.
The data collected from these forms can be very helpful for us as teachers. It can help you to grade more fairly on group assessments, provide written documentation in cases where a student has contributed nothing or very little, and it can help you to figure out which students work best together. Best of all, it lets your students know that you care about them, their feelings, and what they are going through.
