These are small (4 to a page) sheets where students can rate both themselves and their partner or team on how well they worked collaboratively together.
Students work collaboratively a great deal in my classroom, Especially Kagan structures, and they needed to improve some aspects of their partner and team work behaviors. I created these self/partner and self/team evaluation forms so that students can see how others rate their behavior and contributions and to give them a chance to honestly reflect on their own collaborative work. Class time is precious though, so these are sheets that can be used once and awhile and take only a moment for students to fill out.
Sometimes, no matter how many times you tell students they need to use kind words or they need to do their share of the work, it takes the voice of a peer for the behavior to change.
There are two versions of each form- one uses a number system (1-3) and the other uses (Y, M, N). Whichever works for your students.
