When faced with an angry/frustrated parent who claims their child is being bullied, or students themselves who allege being bullied, it becomes the responsibility of the school to investigate.
This form was created to help guide school staff in those investigation. The first part of the form explains the difference between bullying and teasing/being mean. The second part is basic information-gathering regarding the allegations and names of other students/witnesses who may be able to provide additional information.
If the allegations are substantiated, the third section provides options that the staff member could provide to the reporting student or parent so that they know the school takes bullying and peer conflict issues seriously, and wants to help them find a resolution quickly.
