I hope that you find it as useful as I have! This resource is intended for taking observational behavior data on a student who struggles with peer interactions when frustrated. I recommend taking data, using this form, for two weeks to form a baseline. Then, implement interventions in order to decrease negative interactions and increase positive interactions. You may also be able to identify patterns of behavior. Progress monitor the student, by using this form, once a week, after the implementation of an intervention, to see if negative interactions have decreased.
