For a lot of my kiddos with autism, their goals include knowing personal information, such as their name, school, phone number and address.
As you know, visuals are a great help! But the multi-sensory cues of identifying the answer, picking it up and placing it under a "symbol" for the question will really help them understand which question you are asking and remember their answer.
I left the boxes blank, as only you know your client's names. You can simply use a sharpie marker and fill in the information (1 correct, others incorrect) and THEN laminate.
