This packet is designed to be a grab-n-go screener and progress tracker for phonological processes. It can be used as an initial screener during assessment, and as a progress tracker throughout the year for progress notes & report cards. This packet tests 18 Phonological Processes and includes corresponding stimulus picture cards for each process tested! The following processes are assessed: Velar Fronting, Palatal Fronting, Stopping, Final Consonant Deletion, Initial Consonant Deletion, Cluster Reduction, Coalescence, Syllable Reduction, Affrication, Backing, Deaffrication, Alveolarization, Depalatalization, Labialization, Denasalization, Prevocalic Voicing, Postvocalic Devoicing, and Gliding
Pages 4-5: Screening charts
Pages 6-10: Stimulus pictures in color
Pages 11-15: Stimulus pictures in black & white
Pages 16-32: Individual Progress Tracking Data Sheets
Print off and laminate the stimulus picture cards of your choice (black & white, or color). You should only have to do this once as the stimulus cards can be used over and over. I print mine back to front in order to limit the number of pages I need to carry around! If you want to do a phonological process screen (as opposed to tracking progress), print the screening charts (pgs 4-5). I have organized the charts so that the most common processes are contained on the first page. If you want a very quick screen, use the first sheet to test just the most common processes. If you are testing all processes, I recommend printing the screening charts back to front for a quick and easy assessment. If you are tracking progress, you can use either the screening charts or the individual progress tracking sheets. Simply print off the one(s) you need. To screen or assess progress, just show the child the stimulus pictures for the corresponding process that you wish to test, and have them name the picture. Write their response in the space provided on the data sheet, and tally their percentage. That’s it!
