This is a super simple activity that includes two different worksheets which can be used individually or together.
1. This worksheet is a think, pair, share that can be used for students to record their ideas, as well as other students' ideas of what a physical and chemical change is. During the 'share' portion of the lesson, teachers are to clarify and explain. You could always follow up this activity with your own resource (ie. a powerpoint slide listing the different properties etc.).
2. This worksheet is useful for identifying physical and chemical changes. Always make sure to go through the answers with the kids! This is a great resource that can be used for homework - they can use websites for help or not. Lots of flexibility on your part (eg. you can tell them they can use two different coloured pencils to colour the whole box, or use a highlighter to highlight the perimeter of the box, or just highlight the text... the list goes on!)
You can use these worksheets together, or separate. They are one PDF but remember you can choose to print out only one the pages.
