Page 3 – Substitute Teacher – Checklist: Items to Leave for Your Substitute Teacher
When getting ready for a substitute teacher you don’t want to forget anything important! Use this checklist to help you prepare for your day out of the building.
Pages 4-5 – Substitute Teacher – About My Classroom
Your substitute will need to know the basics of how your classroom is organized and run. Photocopy and print this form and re-use each time you have a substitute teacher.
Page 6-7 – Substitute Teacher – Classroom Management for My Substitute
You can share the basics of your management system using this form.
Page 8 – Substitute Teacher – Penalty Box
If your students need a visual reminder of good behaviors when there is a substitute teacher, you can photocopy and cut out these Penalty Boxes so that each student has one on their desk. It would be a good idea to explain the Penalty Box to your students before you have a substitute. That way when the see it, they will already know what it is, which will make the substitute’s job much easier!
Page 9 – Substitute Teacher – Student Accountability Chart
You may want to use this form to remind students that while you are out, you still have expectations that they must follow. This form can help keep your students on track in your absence. Your substitute can pass the forms out at the start of class after reviewing expectations for the class period. As the class is in session, the students should fill out parts of the chart as appropriate. There are multiple entry boxes so this sheet can be reused throughout the day to gather student feedback.
Page 10 – Substitute Teacher – End of the Day Review
Leave this form for the substitute teacher to fill out at the end of the day. It is helpful to have feedback from the sub, and this will guide them to leave the specific details teachers want to know about! There is space to review two classes per page. Photocopy as needed.
Page 11– Substitute Teacher – Student Evaluation
This form can be used as a warm-up the day you return after having a substitute teacher. Hearing what happened from the students’ eyes can help you learn how the day really went!
Page 12 – Substitute Teacher – Emergency Plans
Hopefully you will never have to use this—but in the event you have an unexpected emergency, you might want to pre-plan something for your students to do while you are gone. Use this form to organize a week’s worth of plans just in case they are ever needed.
