This sheet is designed to use as part of problem solving, counseling, and social skills settings. Students can use this both before and after negative interactions with others or situations.
Use the example sheet to show and model how to complete the sheet with students.
Some students may need a scribe to support their use of this sheet.
Laminate this sheet so it can be used multiple time with wet/dry erase markers. Take photographs or photocopies of completed sheets if necessary for documentation and recordkeeping purposes.
Consider using this sheet as part of a behavior intervention plan, RTI/MTSS, and/or IEP supports and data.
Be sure to use this sheet only when the student has some perspective on the issue(s) at hand and is calm. This sheet is helpful when a child can process past concerns and look to the future. If this is used during the problem, it’s likely this won’t be helpful.
Present completed sheets to students when a problem reoccurs to help them make a choice about how to solve a problem.
Be sure to retain copies of this sheet for data collection purposes.
