This is a three page document. The first page is the lesson plan and the accompanying two pages contain the graphic organizers for the lesson. This lesson can and should be repeated every month or so, or at the very least every time you read a new story or book with the students.
The lesson plan includes aim, do-now, instructional objective, mini-lesson, definitions, questions for class discussion, activity to ensure understanding, and summation.
The graphic organizer / handout I find to be very useful because it goes the extra mile. Yes, it requires the students to use evidence from the text but it also requires them to think out of the box (literally since there are boxes on the organizer) and try to figure out how the plot of the text impacts the individual characters.
