This is a double sided, post evaluation that I have students fill out after they finish a unit exam. The first side is to be done after they finish the test. It has them reflect on the ways in which they studied and how much time they devoted in the unit. This method does a good job of show casing all of the possible method for students to prepare for an assessment. The back side is to be completed after they receive their test back and self-correct their test. Once they have a good idea of what they made mistakes on, I have them fill out the back side which has them highlight their strengths and weaknesses on the assessment. Then it asks them to discuss methods for improvement on the next unit. Finally, the student takes the sheet home and reviews it with their parents and they sign off on it. It is a good teacher-parent communication tool and helps increase students ownership of learning. I have left this in Microsoft Word format so that you can modify it to meet the needs of your kids. This could be modified for any subject class.
