For SLP's, obtaining a thorough portrait of a student who has been referred is critical, but often difficult when we are given on perfunctory descriptions of a student's difficulties. This 2-page form is meant to assist with information collection. When teachers request a referral for speech-language testing, give them this form to summarize specific referral concerns, attendance, classroom-based assessment data, literacy- and math-specific strengths & weaknesses, & interventions attempted. The second page is a checklist of the various speech-language areas where teachers can identify difficulties in communicative behaviors.
