These are data sheets I use to keep track of where my students are with their academic goals. They may not work for everyone but I like them! With the letters and numbers I circle it if they identify or produce the sound. For colors and shapes I use a tally mark system I actually got from the speech paths here in the building: a vertical line for a correct response and a horizontal line for an incorrect response. I then take the data and put it into a spreadsheet (posted separately). When it is time to plan or do progress reports all I have to do is open the spreadsheet and walah my data is ready to go.
