I’ve used these forms in my own Special Education Preschool classroom.
Printable Reminder Form - Print on colored paper and write students’ names, date, and time of their conference.
Schedule - Helps you organize your schedule when setting up conferences. You may also hang a copy on your door for parents who are waiting in the hall.
Progress Report - A parent-friendly report that shows what their child can currently do and things they can work on. Print and check off data for each student.
Preschool and Kindergarten Skills to Practice at Home - Highlight areas of concern that parents can supplement at home. For more advanced students, use the kindergarten readiness form.
Conference Notes - Record notes about a student’s progress before your meeting or use this form to make reminders during the conference.
