This daily behavior log is designed for use in preschools or daycare for children ages 4 and up. Oftentimes teachers or caretakers struggle to communicate how a child's day went when parents pick up at the end of the day. This chart breaks down a child's typical day from 8:30 am-5:00 pm into 60 and 90 minute chunks. Each time frame has plenty of space for the teacher to write in the activity that is happening each chunk (i.e. Circle Time, Reading, Centers, Math, etc.)
The goals in this chart are:
I kept my body in the group.
I used whole body listening.
I followed directions.
Each of these targeted behavior goals can be rated by the teacher on a point system. Teachers simply circle the number that most closely describes the child's behavior each chunk of their day.
3 = Student showed the desired behavior 75% of the time or more (Most of the time).
2 = Student showed the desired behavior 50% - 74% of the time (Some of the time).
1 = Student showed the desired behavior 0% - 49% of the time (A little of the time).
At the end of the school day the teacher adds up the points the child earned and checks the box to indicate overall how the child's behavior was that day.
7-36 points = Great Day! (75-100%)
20-26 points = Good Day (55-74%)
0-19 points = Not My Best Day (0-53%)
