Teachers: Use this questionnaire to give a more descriptive explanation of a problematic behavior. When we know more about the behavior we are better able to identify the motivation and therefore create a solution to stopping the behavior. This questionnaire is similar to an ABC (antecedent, behavior, consequence) chart but more direct and easier to answer.
Parents: If your teacher tells you that your child is repeatedly engaging in a problematic behavior you need to know details. Give this form to your teacher to fill out so you are more informed about your child. Together you can solve the problem.
