Students paste this page in the back of their notebooks to track their progress. After students receive feedback on their Integrated Performance Assessments, they put an X in the boxes for the skills that they were assessed on in that IPA.

Reading and Listening only have 3 slots each because I usually only use one interpretive skill or the other for the purpose of time and simplicity. IPA #3 is around the midterm, however, so I try to assess both.
I use the level slots on the right (NM=Novice Mid, IL=Intermediate Low) for student portfolios: when students earn a badge for the designated level in a skill, they get a sticker to put in that level. However, you could also use it to track IPA progress, awarding students a "proficiency badge" (or any sticker) when they have performed at a given level at least twice.
