This is a double-sided worksheet that can be used as practice or as an assessment.
Students write their own verbal descriptions of a proportional relationship. A word bank with units of measure if given to help students be more creative.
From the verbal description, students create a table and a graph, and write an equation. Students also calculate the unit rate and identify the constant of proportionality.
On the second side students answer questions, such as "How do you know it is a proportional relationship?" and "How did you calculate or identify the unit rate?". Students also give examples of a table, graph, and equation that do NOT represent proportional relationships.
This is an excellent way to assess student understanding of proportional relationships while also allowing students to be creative and use topics that interest them.
As an extension, students can present their proportional relationships to the class.
