This is a foldable that covers the 3 forms of equations of a quadratic: standard form, vertex form and factored form. The student can very easily put the foldable together by folding and making a few cuts.
The 3 forms of a quadratic are labeled on the outside. The inside of the flaps are labeled with the corresponding equation and pertinent information for the student to fill in such as what "a" or (h, k) tells us about the quadratic. In addition, each equation includes an example where the student uses the equation to find the pertinent information and then graphs each parabola.
