This is a 3-page test with 26 questions over quadratic functions.
On questions 1 - 5, the student is given a quadratic equation and asked to find all pertinent information such as the vertex, y-intercepts, axis of symmetry, domain & range, etc.
On questions 6 - 11, the student is given a graph of a quadratic application and asked to identify the maximum height, how long the ball is in the air, a reasonable domain & range, etc.
On questions 12 - 14, the student is given a quadratic equation and asked to identify the transformations that have occurred as compared to the parent function.
On questions 15 - 17, the student is given an equation or a table of values and asked to determine if each one represents a linear, quadratic or neither function.
On questions 18 & 19, the student is given a quadratic equation in vertex form, asked to identify the vertex and convert the equation to standard form.
On questions 20 & 21, the student is given some transformations and asked to write the equation of the quadratic.
On questions 22 & 23, the student is given a quadratic equation in vertex form and asked to identify the vertex, whether the graph opens up or down, to sketch a graph of the quadratic and then to find the domain & range.
On question #24, the student is asked to write the equation of the quadratic given its vertex and a point on the graph. Steps are included on this question.
On question #25, the student is asked to complete the square in order to identify the vertex. Steps are included on this question.
Finally, on question #26, the student is given a quadratic application including equation and asked to identify the domain of the situation.
2 different forms of the test are included as well as a key for both.
