Our county has moved more towards "standards based" discussion with parents during Parent Report Card Conferences. I made this form to guide my thinking when talking with parents. I highlight the items the student could use more practice with, along with listing transfer words and sight words they may not know yet or need more support with. I have always done this with my kiddos, but now the form is geared by the Common Core. I have always recieved great feedback from my parents when using this form.
I have attached it as a WORKING document so that if your county does some things a little differently then you have the ability to go in and change things around. With it being a working document you may not have the fonts that I use, so please double check that all information is showing up in the text boxes before printing. If you do not use/have word, I have this document available in pages (It is actually what I use to create so it looks better but it is not supported by the upload). Just send a quick email to firstname.lastname@example.org if you prefer the pages version. After verifying your purchase, I will gladly send it to you!
** The preview pic is not showing the table that the reading levels are in. You can view a PDF version of the document by clicking on the Preview.
The only thing that I ask is that you leave my copyright info on the bottom of the page.
