PLEASE view the preview because the thumbnails are coming out funny looking. (It is similar to my Quarter 1 Chat form!)
Our county is moving more towards "standards" based discussion with parents during Parent Report Card Conferences. I made this form to guide my thinking when talking with parents. I highlight the items the student could use more practice with, along with listing letters/sounds/sight words they may not know yet. Then I give suggestions for how they may be able to help their student at home in the "comments" section on the back. I have always done this with my kiddos, but now the form is geared by the Common Core. I have always recieved great comments from my parents when using this form.
I have attached it as a WORKING document so that if your county does some things a little differently then you have the ability to go in and change things around. With it being a working document you may not have the fonts that I use, so please double check that all information is showing up in the text boxes before printing. If you do not use/have word, I have this document available in pages. Just send a quick email to firstname.lastname@example.org if you prefer the pages version. After verifying your purchase, I will gladly send it to you!
The only thing that I ask is that you leave my copyright info on the bottom of the page.
I hope this document can increase the smoothness of your Parent Conferences.
