I'm sure you agree that having prompt, consistent, and open communication with parents is critical to student success within your classroom.
This note is for those days when things just aren't going quite right for one of your students & parent contact is necessary. This form is quick & easy so you can fill it out & send it home in a backpack, even on those crazy days when you're short on time! Just fill in the student's name, the date, and choose from a list of expected classroom behaviors (there is room for a comment if needed). I ask parents to sign & return these to school so that I know the topic has been addressed at home.
Tip: print a stack of these notes out ahead of time on colored copy paper & keep them in your desk where they are ready to fill out at the end of each day.
