Data, data, data! Teachers are always collecting data and looking for easy ways to quickly record student progress, particularly in large or small group settings.
This formative assessment form has a place for the date, activity name and the goal/criteria that students are aiming to meet in the activity. As students work on the activity the teacher or paraprofessional leading the group adds the student name in the box that matches their skill level (Got It, Needed Help, Not Yet, or Absent). There is a section for the teacher to review the data and make comments or plan next steps based on the recorded progress during that particular activity.
This file contains four identical copies of the form for teachers to make double sided copies in the quantity they need to monitor student progress during group activities.
