I use this form all year for my intervention reading groups in my classroom and have used similar forms as a special education teacher and Title 1 teacher. Where do you go to find out if a student missed more instruction than received if you don't have an attendance chart? How do you track what big ideas each student and their group are getting or struggling with? These forms are easy to stop and jot each time you meet and keep in weekly order so you can refer to them all year long. Next time you must go to a meeting about what's going on with one of your students, you will have all the documentation you need!
