As a classroom teacher, it can be quite challenging to collect data on students that may need additional intervention(s) to grasp the concept being taught in the classroom or assist with behavioral challenges. This weekly hard copy form provides the opportunity to implement RTI tier level intervention(s) two or three times a week and quickly document the results of the response to the intervention(s). A hard copy increases the likelihood of documenting the process for six weeks, which is typical for collecting data in RTI for this amount of time. Writing the goal on the form will guide teachers to the appropriate intervention(s) to help student(s) be successful in school. This form can be implemented in grades K-12.
