This is a 3-page summative assessment over radicals and the quadratic formula. The first 6 questions ask the student to simplify the radical (one of them can't be simplified). The next 6 questions ask the student to add or subtract the radicals (one of them cannot be added). Some simplification is required first.
The next 9 questions ask the student to multiply or divide the radicals. There is one distributive property problem and one problem where the radical is squared. Questions 22 and 23 ask the student to solve the quadratic equation by using the quadratic formula. One of the equations is not set equal to zero and one of them has no solution.
Question #24 asks the student how many solutions are possible when solving a quadratic equation. Finally, there are 2 bonus questions containing a binomial times a binomial (containing radicals) and a really difficult simplifying a radical question. 24 questions total. Key included.
