Love this idea, just wish I would have thought of it myself! Print this form and laminate it to the outside of a manila envelope and laminate. Write your students names, or just go by the number, and fill in the chart as you receive money and permission slips. Stick those items in the envelope to keep it all together. I even left a "total" section at the bottom so that if you choose to write numbers in the boxes (how much money was brought, how many chaperones are attending, etc.) You can tally it all up. Use a vis a vis and you can wipe off to use over and over! Just a great way to organize for field trips by keeping everything together and helping you see who still needs what.
