I use this in my 4/5 classroom. I've tried using notebooks for the kids to journal their reader responses, but it was really unorganized and time-consuming for kids and me. This template is easy to use and keep track of. I print it each week and have the kids put it in their planners in a plastic sheet protector. I check it each morning, write the points they earned, and collect it after a week. They're given it one Monday and I collect it the next Monday.
It is a word document so the days can be changed, as can the expectations (i.e. one complete sentence for younger kids, etc.)
