This freebie has all seven of the forms I use for my students' Reader's Notebooks!
-Reading Goals - Goal setting sheet to help you and your student create reading goals
-Books to Read - A place for students to write down the titles and authors of books they want to get
-Reading Interests - Use this informal interest inventory to keep tabs of who likes to read what!
-Picture Books vs. Chapter Books - The perfect place to track student growth from picture to chapter books throughout the year
-Genre Tracker - Make sure your students are reading widely by using this genre bar graph.
-Reading Log - My tried-and-true reading log
-Notebook Rubric - Use this rubric, with options for teacher and student reflection, to assess reader's notebooks every marking period.
In my classroom, each student has a 1 inch binder to use as a reading notebook. I've also included these forms in a data binder before, and it works equally well. These are perfect artifacts to use during parent conferences!
I've also included FULL instructions on how to use each of the forms, so all you need to do is download, print, and go!
