All of my students have a Reader's Response notebook which they write into during Work on Writing during Daily 5. I collect each notebook once a month and use this rubric to assess their notebook.
The students are assessed on the number of entries they have completed for the month and the organization of their notebook. Students also choose one entry which is assessed for depth of thought, the use of evidence and support, and conventions.
The rubric is in student friendly language and is out of 24 points total.
