Book Log for School: My students book shop once a week. They fill in the form with the book titles and then tally how many times they’ve read each book throughout the week. Helps you to gauge their reading stamina, book choice, and fluency rates.
Reading Log for Home
My students bring home 1 or 2 books to read every night. They fill out this form and get it signed by an adult as homework.

I write in each student’s name in a different square and then make copies. I use this to make sure I confer with each student at least once every week and to jot my notes and observations.
